Again, similar to my other comments. Try giving away the mystery about the stranger and the other woman. Sounds like Sylvia is tthe protagonist, but who is her opponent? What genre is this? Sounds like it could be a romcom with the outrageous coincidence of meeting the doctor. But thenit sounds like a mystery also. I feel like there is lots to this idea, but you need to get to the meat of it in about 25 words. It's a very interesting idea, though.
